Your table time will consist of but not limited to: Tailored techniques, PNF stretching, active release, dry needling, reassessment of mobility etc. Please be considerate and respectful of my time, your time and other patients time. It is good manners to turn up on time and leave on time. If you would like extra time please book it in.
Mind & Body Balance is a full health fund provider however Health Fund rebates varies and depends on type of cover with individual Health Funds and their Policy. Please check with your health fund prior to booking to see if you are eligible. On the spot claiming through HICAPS is available. Cash, Debit card, MasterCard and Visa accepted.
A cancellation fee will apply if you cancel your appointment within 24 hours notice of your treatment time (for treatments of 60 min or less.) 48 hours notice (for all treatments of 90 min or longer) or if you fail to show up, total fee incurred will be 100% of your treatment price.
All oils used are of the highest quality and can include essential oils at the practitioners discretion (essential oil use will be discussed with the patient prior to use). Patients are required to disclose any allergy information to their practitioner.
Please arrive 5min / 10 min earlier than your booked treatment time to fill out your new client form. You will find the consultation form at the reception desk. If Iam unable to greet you at reception. Please take a seat and start filling out your form.

Patients are required to update their practitioner of any allergies, injuries, illness, medications, medical conditions or any relevant changes in their physical condition on the day. Individual health status alters frequently and can change or render treatment. All changes to contact details must be updated.
Patients acknowledge that practitioners of Mind & Body Balance have no responsibility or liability for any damage, injury, loss or otherwise which is caused through use of the practice services, treatment, equipment or facilities, regardless of the cause.
